Deborah Johnson Ingram
Director, Healthcare Compliance & Practice Operations
Primary Care Development Corporation

Deborah Johnson Ingram is the Director of Healthcare Compliance & Practice Operations at PCDC. She joined PCDC in 2010 as a quality improvement specialist with years of experience in HIT adoption and clinical performance improvement program management. As a subject matter expert in practice transformation and patient centered medical home, Deborah has been able to share her expertise and consult with a variety of healthcare organizations to achieve NCQA PCMH Recognition, TJC Primary Care Medical Home Accreditation and CMS Meaningful Use attestation.

Prior to joining PCDC, Deborah was with the New York State Quality Improvement Organization, IPRO where she translated federal quality improvement programs (PQRS) into practical operational and clinical activities in healthcare facilities across the state. Deborah is a certified PCMH content expert (CCE) and she was published in HIMSS’ recent book Medical Informatics, An Executive Primer (2nd edition). Deborah received her Bachelor’s in Sociology from New York University and is a MPH candidate at SUNY Downstate.

Daren Anderson, M.D.

Dr. Daren Anderson is the Director of the Weitzman Institute and VP/Chief Quality Officer of Community Health Center, Inc. In this role, Dr. Anderson is responsible for ensuring that CHC delivers the highest possible quality of care to its patients, developing a strong quality improvement infrastructure across CHC, promoting research and development, and supporting CHC’s mission to become a nationally-recognized center of world-class healthcare

Previously, Dr. Anderson has served as Director of Primary Care for the VA Connecticut Healthcare System, Assistant Professor of Medicine at Yale School of Medicine, Chief Medical Officer of Community Health Center, Inc., a consultant in the field of disease management, and a primary care provider at the Community Health Center of New Britain.

Dr. Anderson obtained his undergraduate degree at Harvard College and his medical degree from the Columbia University College of Physicians and Surgeons. He completed his residency training in internal medicine at Yale-New Haven Hospital and is a board-certified general internist.

Dr. Anderson researches, lectures and writes extensively in the areas of disease management, self-management/behavior change, implementation science, and health services research. He has published articles in several top-tier peer-reviewed medical journals, including the Journal of General Internal Medicine, Archives of Internal Medicine, and Diabetes Spectrum. He is the 2010 recipient of the Society of General Internal Medicine’s Hamolsky Award for his research on disease management for diabetes in a community health center.

Presentation: Quality Improvement Strategies for Success

Patti Feeney
Managing Director, Quality Improvement Education
Weitzman Institute

Patti Feeney is the Manager of Quality Improvement Education at Community Health Center, Inc.’s Weitzman Institute. Patti joined CHC in the Human Resources Department in 2005 with a background in Human Resources Management, Customer Service Management, Training and Quality Improvement. Patti transitioned into CHC’s Quality Improvement Department and assisted in the development of the Weitzman Institute in 2011. Patti has experience in Lean improvement methodology and earned her Lean Six Sigma Green Belt and Six Sigma Black Belt certification and is also certified through the Dartmouth Institute as a Clinical Microsystems coach. Patti coaches multiple Mesosystem improvement teams at CHC and manages QI training initiatives which include key improvement strategies and methods, facilitation, team dynamics, change management, implementation science methods, sustainability and measurement. Patti has her Bachelor’s Degree in Corporate Communications from Southern Connecticut State University and Master’s Degree in Business Management from Albertus Magnus College.

Dr. Mark Splaine, MD, MS

Mark is the Director of Education at Community Health Center, Inc.’s Weitzman Institute. Mark joined CHC in 2016. Before joining CHC, Mark worked as an independent consultant in healthcare quality education and measurement for EduHealth LLC. Mark is professionally trained as a general internal medicine physician and practiced primary care for 16 years. Mark has been a teacher of health professional learners including physicians, nurses, pharmacists, and health administrators for over 20 years. Mark received his Bachelor of Arts degree in Biochemical Sciences from Harvard College, his Doctor of Medicine degree from Dartmouth Medical School, and his Master of Science degree in the Evaluative Clinical Sciences from Dartmouth College.

Presentation: Integrating Quality with Diabetes Workflow

Dr. Bruce Block, MD
Chief Learning and Medical Informatics Officer, Pittsburgh Regional Health Initiatives; Director, PRHI-PA REACH West Project, Yale School of Medicine

As Chief Learning and Medical Informatics officer for the Pittsburgh Regional Health Initiative, Dr. Block supports the implementation of electronic health records (EHRs) and quality improvement methods in medical practice. He is responsible for PRHI’s PA REACH West initiative, which assists physician practices with EHRs and helps them achieve meaningful use and medical home status. Covering a territory stretching from Erie to Greene County, PRHI’s REACH initiative serves more than 850 providers in more than 350 practice sites. To date, 99 percent of PRHI’s REACH clients have implemented EHRs, and 92 percent have achieved meaningful use status.

Dr. Block began practice as a family doctor in rural western Pennsylvania in 1972. In 1981, he joined the faculty of the Shadyside Family Medicine residency program. In 1985, he developed and implemented an EHR at the Shadyside Family Health Center practice. In 1998, he co-founded the Centers for Healthy Hearts and Souls, a nonprofit health promotion organization in partnership with more than 40 African-American churches and community organizations. After 15 years as the medical director of the Shadyside Family Health Center, Dr. Block created the Shadyside Primary Care Institute where he developed quality improvement software for physicians, researched and published articles about practice improvement and community health. He is clinical Professor of Family Medicine and adjunct Professor of Biomedical Informatics at the University of Pittsburgh, School of Medicine.

Presentation: Steps for Increasing Colorectal Screening Rates in Community Health Centers

Dr. Durado Brooks, MD, MPH
Managing Director
Cancer Control Intervention, American Cancer Society

Dr. Brooks has worked for the American Cancer Society’s national headquarters since 2000. He is involved in the creation and implementation of strategies to improve the prevention and early detection of cancer and to reduce cancer disparities. He also serves as Deputy Editor of the Society’s peer-reviewed journal CA: A Cancer Journal for Clinicians.

A graduate of the Ohio State University and the Wright State University School of Medicine, Dr. Brooks completed his internal medicine residency and chief residency at Wright State’s Affiliated Hospitals in Dayton, Ohio. He completed the Commonwealth Fund/Harvard University Fellowship in Minority Health Policy in 1999 and holds a Master’s degree from the Harvard School of Public Health. Before joining the American Cancer Society Dr. Brooks practiced primary care medicine and served as medical director in community health centers in Ohio and in his current hometown of Dallas, TX.

In 2015 Dr. Brooks was honored with the Prevent Cancer Foundation’s Laurel Award for National Leadership in cancer prevention and early detection.
